The Robert J. Dole VA Medical Center, a designated Pathway to Excellence facility by the American Nurses Credentialing Center, is looking for a Registered Nurse to join the Dodge City Community-Based Outpatient Clinic (CBOC). You'll provide direct care and care coordination for Veterans in the Primary Care Department, managing patient panels, promoting health and wellness, and working within the Patient Aligned Care Team (PACT) model. Here's what you'll be doing:

• Providing direct Veteran care and care coordination within the Primary Care Department.
• Managing assigned patient panels, utilizing registries, and ensuring appropriate evaluation and access for Veterans.
• Delivering health promotion, health education, coaching on wellness, disease prevention, and chronic care management.
• Collaborating in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
• Demonstrating leadership to improve care for the Primary Care Department and facility.
• Providing formal and informal constructive feedback to peers and supporting colleagues through knowledge sharing.
• Fostering a safe, supportive environment for the professional development of healthcare professionals.
• Evaluating outpatient outcomes of evidence-based decisions and practice changes at the program or service level.

What you'll need to have:

• U.S. Citizenship is required (non-citizens may be appointed when qualified citizens cannot be recruited).
• Graduate of a school of professional nursing approved by ACEN or CCNE.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory, or the District of Columbia.
• Proficiency in basic written and spoken English.
• Must pass pre-employment physical examination.

Preferred Experience:

• Primary Care nursing experience.
