Description

The Robert J. Dole VA Medical Center is hiring a PRN Registered Nurse for the Salina Community-Based Outpatient Clinic (CBOC) in Kansas. This is an intermittent position, meaning you'll work based on clinic needs. You'll be part of a Patient Aligned Care Team (PACT), providing direct care and coordination for Veterans in primary care. Here's what you'll be doing:

• Providing direct patient care and care coordination for Veterans within the Primary Care Department
• Managing assigned patient panels and utilizing registries to ensure appropriate evaluation and access
• Delivering health promotion, education, and coaching on wellness, disease prevention, and chronic care management
• Collaborating on the development, implementation, and revision of policies, procedures, and clinical guidelines
• Supporting colleagues through knowledge sharing and providing constructive feedback for improvement
• Evaluating patient outcomes based on evidence-based decisions and practice changes
• Documenting PACT outcomes at the program or service level
• Fostering a safe environment that supports the professional development of healthcare team members

What you'll need to have:

• Graduate of an accredited school of professional nursing (ACEN or CCNE accredited program)
• Current, full, active, and unrestricted RN registration in a U.S. state, territory, or the District of Columbia
• Proficiency in basic written and spoken English
• U.S. citizenship (non-citizens may be considered when qualified citizens are unavailable)
• Ability to pass a pre-employment physical examination and drug screening
• Primary Care experience preferred
