Want to bring healthcare directly to veterans who need it most? The Puget Sound VA Health Care System is hiring Nurse Practitioners for their Home Based Primary Care program within Geriatrics and Extended Care. You'll travel to veterans' homes, providing the full scope of primary care services to those who can't easily make it to a clinic. This role lets you practice to the full extent of your education and training, with prescriptive authority and real autonomy over your patient panel. Here's what you'll be doing:

• Independently managing a panel of patients with acute and chronic health conditions, focusing on health promotion, disease prevention, and maximizing function
• Traveling to veterans' homes to deliver primary care services using a government vehicle
• Developing and implementing individualized care plans for optimal population health and disease management
• Ordering and interpreting diagnostic studies and prescribing pharmacologic and non-pharmacologic interventions
• Working collaboratively with a multi-disciplinary team and initiating referrals to specialty services
• Assessing seriously ill veterans' goals and preferences for care and quality of life
• Providing education to patients and their families while continuously adjusting care plans as needed

What you'll need to have:

• U.S. Citizenship (non-citizens may be appointed when qualified citizens cannot be recruited)
• Post-master's certificate or master's/doctoral degree as a Nurse Practitioner from an ACEN or CCNE accredited program
• Active, current, full, and unrestricted APN license to practice as an NP in any U.S. state or territory
• Current board certification as FNP, ANP, AGPCNP, or GNP from ANCC, AANPCB, or other nationally recognized certifying body
• Proficiency in basic written and spoken English
• Valid driver's license (required for home visits)
• Ability to meet physical requirements including heavy lifting (45+ lbs), walking, standing, and repeated bending
