The Richmond VA Medical Center is looking for a Geriatric Physician to serve as Medical Director of its well-established Transitional Care Program. This program has been operational for nearly 20 years, and you'll be joining a collegial group of physicians and advanced practice providers who genuinely embrace work-life balance. You'll practice in inpatient, outpatient, and telehealth settings while leading an engaged interdisciplinary team. Here's what you'll be doing:
- Provide day-to-day clinical care in the Geriatrics Transitional Care Program, geriatric clinics, and telehealth modalities, with coverage in other GEC programs as needed.
- Lead and collaborate with the interdisciplinary team, referral sources, and stakeholders on a daily basis.
- Supervise clinical staff including a nurse navigator, nurse practitioner, physician assistant, and administrative personnel.
- Participate in program development, quality improvement, Systems Redesign projects, and root cause analyses.
- Engage in educational activities for Geriatrics and HPM fellows, Internal Medicine residents, and medical students, including curriculum development and bedside teaching.
- Assist the service chief with administrative duties such as timekeeping, recruitment, staff development, and regulatory inquiries.
What you'll need to have:
- U.S. Citizenship (non-citizens may be considered when qualified citizens cannot be recruited).
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an approved institution.
- Current, full, and unrestricted medical license from any U.S. state or territory.
- Completed residency training approved by ACGME or AOA, with eligibility for board certification.
- Board eligibility or certification in Internal Medicine or Family Medicine (required for trainee supervision).
- Fellowship training in Geriatrics and/or Hospice & Palliative Medicine highly desired.
- Recent and substantial experience in geriatric transitional care and academic hospitalist/internal medicine practice preferred.
- BLS certification (renewed every two years) and DEA registration required.
Job Details
Location: Richmond, Virginia
Schedule: Full-time, Monday through Friday, 8:00 AM to 4:30 PM (shared on-call for after hours, weekends, and federal holidays)
Annual Salary Range
$265,000 - $335,000
Plus annual performance bonus and recruitment incentive for qualified candidates

The Geriatrics and Extended Care (GEC) service at Richmond VAMC offers a comprehensive suite of programs: outpatient geriatrics, Community Living Center/nursing home care, Home-Based Primary Care, inpatient and outpatient Palliative Care, hospice care, transitional care, non-institutional care, and State Veteran Home partnerships. You'll work alongside trainees from Virginia Commonwealth University, including Geriatrics and HPM fellows, Internal Medicine residents, and medical students.
