Rehab CCC Coordinator - University Health Network | Toronto, ON

The University Health Network (UHN) is seeking a Rehab/CCC Coordinator to serve as the clinical lead for education and training related to Rehabilitation (NRS, FIM) and Complex Continuing Care (CCRS/IRRS) reporting. Based at the Bickle Centre, this role is essential for ensuring clinical staff are well-prepared to use key assessment tools, enabling accurate data collection and improved patient care. Reporting to the Manager of Decision Support, the Coordinator will collaborate with clinical teams, site leadership, and corporate partners to promote continuous learning, maintain data quality, and advance operational excellence within Canada’s largest research teaching hospital.

Key Responsibilities

• Design, coordinate, and deliver training programs for interRAI, NRS, FIM, CCRS, and IRRS reporting tools for new and existing clinical staff.
• Develop and maintain training resources, manuals, and online educational materials while providing individualized coaching to assessors.
• Serve as the clinical expert for reporting systems, troubleshooting workflow issues and ensuring data integrity for operational decision-making.
• Oversee data submission accuracy and compliance with CIHI and MOHLTC standards, providing timely data quality feedback to clinical teams.
• Communicate and educate staff on CIHI methodology updates and lead annual Professional Practice Day events.
• Collaborate with Decision Support, IT, and vendors to support system updates, testing, and troubleshooting for data collection applications.
• Engage in internal committees, CIHI forums, and working groups to align education strategies with broader organizational priorities.

Qualifications

• Master’s degree in a regulated health profession is required.
• Current registration in good standing with a recognized regulatory college.
• Minimum 2–3 years of clinical experience in CCC, Rehabilitation, or Long-Term Care.
• Minimum 1 year of experience with interRAI, NRS, FIM, CCRS/IRRS assessment and training (preferred).
• Certified NRS and interRAI/CCRS Trainer (preferred).
• Strong expertise in adult education principles, facilitation, and developing educational materials.
• Excellent communication, organizational, and interpersonal skills.
