Physical Therapist – Home Health

Description

$5,000 sign-on bonus available to external candidates (per Sutter Health policy) plus excellent benefits! Primary coverage area includes Mountain View, Palo Alto, Stanford, Los Altos, and surrounding communities. Responsible for assessing, planning, organizing, and participating in rehabilitative programs aimed at improving mobility, relieving pain, restoring strength, and enhancing functional independence. Serves as a subject matter expert in workflow/clinical protocols, develops department guidelines, and mentors staff and students. Leads continuous staff education programs and ensures compliance with federal, state, and organizational standards to maintain optimal care, privacy, and safety.

Key Responsibilities

• Assess patients’ physical status and develop customized rehabilitative plans of care.
• Provide therapeutic treatments to improve mobility, relieve pain, and restore function.
• Collaborate with staff and leadership to align clinical protocols with best practices.
• Educate, train, and mentor newer staff and assigned students.
• Lead staff education sessions and develop training materials/protocols.
• Ensure compliance with regulatory, organizational, and billing standards for home health care.
• Document patient care according to local/state/federal requirements.
• Participate in workflow improvement, data analysis, and evidence-based program development.

Qualifications

• Graduate of an accredited physical therapy school (required).
• Valid PT – Physical Therapist license (State of California).
• Current Basic Life Support (BLS) certification required.
• California Class C Driver’s License and automobile insurance (Home Health program requirement).
• At least 5 years of recent relevant experience.
• Proven expertise in rehabilitative treatment protocols and home health regulatory compliance.
• Strong critical thinking, decision-making, and problem-solving skills.
• Ability to educate, mentor, and lead training sessions effectively.
• Excellent verbal, written, and interpersonal communication skills.
• Proficiency in technology, documentation systems, and telehealth tools as required.
