Nurse Case Manager

📍 Duties and Responsibilities

• Manage a caseload of clients with chronic or long-term conditions using a client-centered, multidisciplinary approach.
• Conduct functional assessments and create care plans in collaboration with physicians, allied healthcare providers, and community agencies.
• Coordinate and monitor interventions, evaluate care outcomes, and support transitions across the care continuum.
• Develop partnerships with General Practitioners.
• Provide evidence-based clinical direction, guidance, and support to interdisciplinary staff.
• Facilitate client and family education, chronic disease management strategies, and promote self-care.
• Liaise with healthcare professionals, community resources, and education staff to promote patient outcomes and continuity of care.
• Maintain documentation aligned with professional standards and organizational policies.
• Promote ethical, trauma-informed, equitable care.

📍 Qualifications

Education & Experience

• Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• 3 years of recent experience in acute care with a focus on medical/geriatric, or home/community care.
• Valid BC Driver’s License and access to a personal vehicle for local travel.

Knowledge, Skills & Abilities

• Strong critical thinking and case management capabilities.
• Skilled in chronic disease management, psychosocial assessments, and physical examination.
• Knowledge of adult education principles and clinical teaching methods.
• Effective communication, negotiation, and conflict resolution abilities.
• Ability to handle unexpected situations and adjust clinical interventions accordingly.
• Experienced working within interdisciplinary teams.
• Proficient in computer use and health information systems.
