LVN Care Coordinator - Sutter Care at Home, Santa Cruz

Sutter Care at Home is seeking an experienced LVN Care Coordinator to join our Home Health team. In this key role, you will organize and coordinate the overall workflow of LVNs and assistants, while also providing direct nursing care under the supervision of a Registered Nurse. You will be responsible for implementing patient care plans and ensuring the efficient functionality of the department.

Key Responsibilities

• Organize and coordinate daily workflow, including staffing and work assignments for LVNs and assistants.
• Provide direct nursing care under the supervision of an RN or physician, in accordance with the patient's plan of care.
• Integrate education, technical competence, and patient interaction skills to gain cooperation from patients and their families.
• Support departmental functionality by managing supplies and performing equipment maintenance, testing, and setup.
• Adhere to all local, state, and federal regulations to ensure patient privacy and safety.
• Build collaborative relationships with peers and other healthcare providers to achieve departmental objectives.

Qualifications

• Graduate of an accredited school of Vocational Nursing.
• Current Licensed Vocational Nurse (LVN) license in California.
• Minimum of 4 years of recent, relevant experience.
• Valid Basic Life Support (BLS) certification.
• Valid California Driver's License and current automobile insurance.
• Strong knowledge of nursing skills, medical terminology, and computer applications.
