📍 Duties and Responsibilities

  • Lead the HSCL program and ensure alignment with provincial and organizational protocols.
  • Evaluate client eligibility and maintain accurate data.
  • Develop and implement comprehensive care plans in collaboration with the interdisciplinary team.
  • Support and guide clients and caregivers in navigating healthcare services.
  • Participate in inter-agency case management.
  • Address legal, ethical, and professional implications in client care decisions.
  • Provide coaching and educational support to clients, caregivers, and health professionals.
  • Maintain strong collaborative relationships with internal teams and external health/community agencies.
  • Utilize computer systems for documentation and statistical data analysis.

📍 Qualifications

🎓 Education & Experience

  • Registered Nurse (RN) with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
  • A minimum of 3 years of recent experience in community health nursing or a similar clinical environment.
  • Valid BC Driver’s License and access to a personal vehicle for local/open-area travel.

🧠 Knowledge, Skills & Abilities

  • In-depth knowledge of BCCNM Standards of Practice and the HSCL model.
  • Understanding of social determinants of health, community support systems, and provincial legislation affecting people with developmental disabilities.
  • Excellent case management, critical thinking, and problem-solving abilities.
  • Proficient in facilitation, mediation, and conflict resolution strategies.
  • Strong interdisciplinary collaboration and independent decision-making skills.
  • Experience with diverse populations and demonstrates cultural humility.
  • Advanced organizational, prioritization, and time management skills.
  • Skilled in the use of computer systems for documentation, client records, and data collection.
  • Certified proficiency in CPR and emergency response techniques.
  • Physically able to perform the duties assigned in both community and office-based settings.
