Baycrest Health Sciences invites applications for a Nurse Practitioner to support community-dwelling clients aged 18+ through innovative primary care models. Reporting to the Ambulatory Services department, the NP will collaborate with physicians, interdisciplinary teams, and external partners to deliver accessible, integrated care focusing on chronic conditions, geriatric needs, mental health, and physical/cognitive wellness.

This role supports individuals who are unattached to primary care and helps establish those critical links. You will support population health through education, assessment, advanced diagnostics, and therapeutic interventions.


Primary Responsibilities:

  • Provide full-scope primary care (autonomous assessments, diagnostics, treatment, and follow-ups)
  • Manage chronic disease, mental health, and geriatric care in diverse settings
  • Assist clients in achieving health goals through personalized education and programming
  • Use virtual care platforms as part of innovative service delivery (e.g. day hospitals, outreach, NP-led clinics, virtual clinics)
  • Prescribe controlled substances and collaborate on care continuity
  • Participate in program and service model development tailored to the community
  • Advocate for client needs at institutional, system, or policy levels
  • Conduct research and apply evidence-based solutions to clinical challenges
  • Contribute to regional planning and health system innovations

🎓 Qualifications:

  • RN (EC) – Extended Class nursing license (CNO) in good standing
  • Nurse Practitioner-Primary Health Care (NP-PHC) or NP-Adult designation
  • Controlled substance prescribing certification
  • 5+ years experience in geriatric or primary care across two or more environments (inpatient, community, homecare, virtual)
  • CNA certification in Gerontology Nursing (preferred)
  • Strong experience in diagnostic test interpretation, prescribing, and medical directives execution
  • Proven leadership, communication, and research skills
  • Familiarity with Ontario healthcare and regional planning is preferred
  • Strong digital health competencies (webinars, mobile apps, EMRs, Telehealth)
