Clinical Pharmacist – Community Health Services (Part-Time, Nanaimo)

🧑‍⚕️ Role Overview

• This Clinical Pharmacist serves patients in Community Health Services including:
  • Home & Community Care
  • Seniors Outpatient Clinics
  • Geriatric Specialty Services
• Provides hands-on, client-centered care through visits to homes and clinics.

🧩 Team Collaboration

• Works collaboratively with:
  • Seniors, End of Life, and Home & Community Care teams.
• Acts as a strategic resource for prescribers and healthcare providers in:
  • Medication management
  • Client assessments
  • Treatment planning

💊 Primary Responsibilities

• Identify and resolve drug therapy problems in home and community settings.
• Provide medication counseling and drug education tailored to seniors and palliative clients.
• Conduct clinic sessions and home visits based on client care needs.
• Deliver academic detailing to optimize evidence-based prescribing practices.
• Ensure client-centered, values-based clinical decisions are made in all interactions.

🤝 Interprofessional Engagement

• Create and maintain working relationships with:
  • Physicians, nurses, case managers
  • Family caregivers and community partners
• Participate in care conferences and planning meetings.

📚 Education & Knowledge Sharing

• Serve as a clinical educator for both peers and care providers.
• Model pharmacy best practices and preventive care planning.
• Support patient and caregiver education with up-to-date knowledge on medication use.

🌍 Practice Environment

• Must be flexible and adaptable to:
  • Varying community care environments
  • Travel between multiple sites as part of standard workflow
• Plays a direct role in improving quality of care for the elderly and palliative populations.
