Position Summary

The Licensed Vocational Nurse (LVN) – Home Health at Sutter Care at Home provides patient-centered nursing care under the direction of a Registered Nurse. This role involves assisting in the nursing process, delivering basic nursing care, and instructing patients and caregivers in self-care to promote independence and quality of life. The LVN collaborates with interdisciplinary teams, ensures patient safety, and maintains professional competencies in a dynamic home health environment.

Key Responsibilities

• Assist in the nursing process by providing basic nursing care and instructing patients and caregivers in self-care.
• Perform patient assessments, focusing on physical, psychological, and social needs, and report findings to the Registered Nurse.
• Deliver care in accordance with established standards, ensuring patient safety and reducing the likelihood of healthcare errors.
• Collaborate with interdisciplinary teams, including physicians, therapists, and community resources, to coordinate patient care.
• Provide education and support to patients and families on the use of adaptive equipment and self-care techniques.
• Document care accurately and maintain patient confidentiality in accordance with organizational policies.
• Handle sensitive issues, manage conflicts, and prioritize tasks in a challenging work environment with time demands.
• Participate in quality improvement initiatives and maintain compliance with regulatory and licensure requirements.

Qualifications

• Graduate of an accredited school of Vocational Nursing or equivalent education/experience.
• Licensed Vocational Nurse (LVN) certification.
• Basic Life Support (BLS) Healthcare Provider certification.
• Valid California Driver’s License (Class C or B) and automobile insurance.
• Minimum 1 year of professional experience, with direct patient care experience within the past 5 years.
• Home health or community health experience preferred.
• Basic competencies in nursing fundamentals and pharmacology.
