Position Summary

The Licensed Vocational Nurse – Encina Concierge at Palo Alto Medical Foundation works under general supervision and within the scope of their licensure to assist providers in delivering personalized patient care. This role involves greeting and identifying patients, escorting them to exam rooms, identifying the purpose of their visit, preparing patients for office visits by taking vital signs and performing minor medical procedures, and other tasks associated with patient care, all in accordance with the California Nurse Practice Act and PAMF Nursing guidelines.

Key Responsibilities

• Greet and identify patients, escort them to exam rooms, and confirm the purpose of their visit.
• Prepare patients for office visits by taking vital signs, weighing patients, and performing minor medical procedures as directed.
• Assist providers with patient care tasks within the scope of LVN licensure and PAMF guidelines.
• Document patient information accurately in electronic health records per organizational standards.
• Maintain patient privacy and confidentiality of protected health information (PHI) in compliance with HIPAA.
• Collaborate with providers, patients, and healthcare teams to ensure a high-quality, personalized patient experience.
• Adhere to organizational policies, safety precautions, and applicable laws/regulations to promote a reliable patient experience.
• Maintain clinical competency through ongoing education and training within the scope of practice.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Vocational Nurse (LVN) license for California.
• Basic Life Support (BLS) Healthcare Provider certification.
• IV Certification upon hire.
• Previous related medical experience preferred.
• Demonstrated knowledge of basic nursing skills, medical terminology, and patient care procedures.
• Strong communication skills to interact professionally with patients, families, and healthcare teams.
• Ability to prioritize tasks and work within standardized policies and procedures.
• Basic knowledge of computer applications, including Electronic Health Record systems.
• Ability to work collaboratively in a team-oriented environment while managing patient care tasks.
